COMPLEMENTARY AND ALTERNATIVE HEALTH CARE CLIENT BILL OF RIGHTS
In accordance with Minnesota statutes you must acknowledge that you have received a copy of the “complementary and alternative health care client bill of rights” prior to the receipt of alternative health services from Jason Stoen / Jason's Office (Jason's Office LLC).

STATEMENT OF CREDENTIALS.
Jason has been working in the allopathic, homeopathic, and naturopathic health care industry for over 25 years. He is the founder and chief consultant for Jason's Office LLC. He works from his home office as a Complementary and Alternative Health Care Practitioner and Certified Natural Health Consultant. He holds degrees in Respiratory Care and Exercise Physiology and is a Minnesota and Iowa Licensed Respiratory Therapist and Sleep Technician. He is certified in Natural Health Consulting and Alternatives in Child Brain Development. Additionally, he has experience and training in Naturopathy (Clayton College of Natural Health), Functional Diagnostic Labs (Adrenal, Hormone, Metabolic, Digestive, Gut Pathogen, Hair Tissue Mineral and Heavy Metals, and Food Sensitivities), Blood Type & Metabolic Type Consulting, Natural Alternatives to Prescription Drugs, FoodPharmacy™, Herbal TCM, Aroma Therapy, Constitutional Analysis, and Classical Homeopathy.
LICENSURE DISCLAIMER.
Although Jason is a licensed health care professional, services related to his professional license are not offered in conjunction with services which fall under the “unlicensed complementary and alternative health care practitioners act”. In other words, you will not be receiving care from a respiratory therapist or sleep technician as part of your natural, functional, and/or integrative health care experience. However, Jason’s background does allow him to better understand the client’s health care needs and experiences, as well as the skill set and connections to effectively facilitate an integrative health care experience.
STATEMENT OF PHILOSOPHY.
Jason’s Office is positioned to bridge the gap between self-care and medical care by engaging clients in a systematic (subclinical) model of care that focuses on natural, holistic, functional, and integrative health care services. Jason’s Office presents these services as a safe and effective solution to both our modern health care crisis, and the many stress-related health conditions affecting humanity today. Constitutional assessments, functional lab services, and a supportive professional network, are just some of the resources used to identify healing opportunities at a root cause level. Emphasis is placed on all-natural protocols, lifestyle changes, and nutritional strategies proven to build health and restore the innate healing potential of the client. Services provided through Jason’s Office are perfectly suited for distance consultation which offers clients a very accessible and affordable alternative health care experience with self-guided, lab-guided, and practitioner-guided options available. Jason's Office makes these services available to other businesses and practitioners of complementary health services that share this philosophy.

Under Minnesota law, an unlicensed complementary and alternative health care practitioner may not provide a medical diagnosis or recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis from a licensed physician, chiropractor, or acupuncture practitioner, or services from a physician, chiropractor, nurse, osteopath, physical therapist, dietitian, nutritionist, acupuncture practitioner, athletic trainer, or any other type of health care provider, the client may seek such services at any time.”

FEES
Current lab fees are available by contacting Jason's Office or your referring practitioner. Jason’s consultation fees are $3/minute and a $90 minimum consultation fee applies. If you are accessing services through another business or practitioner, contact them regarding their consultation fees. Professional grade products are available through Jason’s Office through third party resources.
BILLING PROCESS.
You the client are billed directly for services; no contracts exist between Jason Stoen / Jason's Office
LLC and any insurance company. Insurance generally does not cover our services or labs. You the client have the right to reasonable notice of any changes in services or charges that may occur. Consultation and lab fees are prepaid.
RECORDS AND CONFIDENTIALITY.
You the client have the right to access your records and written information from records in accordance with Minnesota statute 144.335. Your records and transactions are held confidential, unless the release of these records is authorized in writing by you the client, or as otherwise required by law.
SERVICE AND ASSERTING RIGHTS.
You the client have the right to complete and current information concerning recommended services including the expected duration of the service to be provided. You the client have the right to refuse service for any reason at any time, unless otherwise provided by law. You the client have the right to choose freely among available practitioners and other health services. You the client have the right to expect courteous service and to be free from verbal, physical, or sexual abuse. You the client have the right to assert your client rights without retaliation.
ACKNOWLEDGMENT OF RECEIPT.
You the client/client representative must sign that you have received the “complementary and alternative health care client bill of rights” prior to the provision of services.
